Job Summary: The Care Manager will be trained in and deliver targeted case management and evidence-based family-oriented, individual and/or group psychosocial rehabilitative services, skills training, and/or medication training. A Bachelor’s degree in social work, psychology, or any other human services is required.

Responsibilities, Knowledge, Skills, and Abilities:

· Conduct screening with clients, ensuring timely completion of intake paperwork.

· Complete training and certification in all evidence-based programs including Seeking

· Safety, Aggression Replacement Training, Preparing Adolescents for Young Adulthood,

· Defiant Child/Teen, and Nurturing Parenting Program.

· Maintain all certifications valid, renewing training and re-certification annually or as needed.

· Maintain caseload of clients in care, completing also associated administrative and documentation tasks.

· Ensure all daily documentation is completed and entered all required databases in a timely and efficient manner.

· Provide crisis intervention and safety planning for youth and their families as needed.

· Develop and document treatment plans, including goals and objectives, on a timely basis or no later than every 90 days.

· Based on client need, provide skills training, case management, psychosocial rehabilitation and/or medication management services, in alignment with the client’s treatment plan.

· Always maintain client confidentiality and be familiar with all HIPPA laws and regulations

· Participate in multidisciplinary case staffing meetings, training, and supervision as appropriate.

· Works closely with all project staff to coordinate and integrate services.

· Knowledge of social services system and community resources.

· Ability to work as a team player with project staff, families, and various community and education programs.

· Communicate clearly and concisely, both orally and in writing, with partner agencies and families.

Job Type: Full-time

Salary: $45,000.00 per year

Job description

Title: Care Coordinator- Targeted Case Management

Supervisor: TCM Clinical Supervisor

General Qualifications: Must possess a minimum of a bachelor’s degree in rehab counseling, psychology, social work, or related field as specified in the Texas

Position Description: This Qualified Mental Health Professional will provide Targeted Case Management services to clients in care at the El Paso Child Guidance Center. Care Coordinator will therapeutically engage and form partnership with youth, adults and families to empower them towards self-sufficiency. Skills will be applied in engagement, treatment planning, implementation and transition of youth and their families. This professional is also to be trained in and will deliver family-oriented, individual and/or group rehabilitative mental health services, skills training and development, medication training, parenting skills training, and case management services to clients of the agency per state specified standards and requirements

General understanding of adult and child mental health, treatment, and documentation of treatment required. Knowledge of confidentiality and ethical guidelines for practice also required. Should possess basic therapeutic skills, strong verbal and written communication skills, and excellent organizational skills. Coordinator will be further trained in supporting youth and their families in participating in the service continuum of care offered at EPCGC.

Classification: Hourly, non-exempt

Job Responsibilities: Complete onboarding processing, including training and certification in all evidence-based methodologies, not limited to but including Wraparound, when applicable.

  • Become certified as QMHP-CS and participate fully in credentialing process with EPCGC.
  • Maintain caseload of TCM clients in care, completing also associated administrative and documentation task .
  • Conduct CANS/ANSA assessments with clients, ensuring timely completion of all assessment and intake paperwork.
  • Establish and maintain therapeutic relationships with all youth and families throughout service delivery of TCM and/or rehabilitative services.
  • Ensure all daily documentation is completed and entered into all required databases in a timely and efficient manner.
  • Provide crisis intervention and safety planning for youth and their families as needed.
  • Assist in developing discharge plans, goals and objectives while eliminating barriers to accessing recovery services.
  • Actively participate with clients to determine best treatment for recovery. Work involves coordination and linkage activities to community-based services
  • Attend all team, departmental, organizational and professional development meetings and trainings as required and scheduled
  • Maintain client confidentiality at all times and be familiar with all HIPPA laws and regulations
  • Prepare and present all assigned cases for review by a supervisor, colleagues and multi-disciplinary treatment team members, as required on a prescribed basis
  • Participate in interdisciplinary case staffing meetings, trainings, and supervision as appropriate
  • Maintains compliance with all policies and procedures of the EPCGC, including compliance with COA standards and all other regulatory requirements
  • Complete all reporting tasks related to service and program evaluation, including participation in annual PQI processes.
  • Exhibit sensitivity to cultural and socioeconomic differences in the service population using Trauma
  • Trauma-Informed Care (TIC) principles and practices.
  • Operationalize agency values in supporting overall health and well-being of youth and families.
  • Other duties as assigned by TCM Clinical Supervisor and/or Behavioral Health Director.
